Practice information & FAQ
Can I read the consent form before my operation?
Yes. The procedure consent forms are available on the Consent forms page to read in advance. Please do not sign them at home: Dr Labuschagne will go through the form with you, answer your questions and sign it with you at the consultation.
Do I need a referral?
No. You are welcome to book directly without a referral, provided your medical aid allows it, which in the majority of cases is not a problem. Many patients are also referred by a GP, paediatrician or specialist. If you are unsure, please check with your medical aid before your appointment.
How do I make an appointment?
For both Netcare Unitas and Nelson Mandela Children's Hospital, phone the rooms on 012 664 6026 or email reception.jjlab@gmail.com. Please have your medical aid details and any referral letter ready.
What should I bring?
Your referral letter, previous reports and scans (discs or links), a list of medication, your medical aid card and ID. For babies, please bring the Road-to-Health booklet.
Which medical aids do you accept?
The practice is contracted in to several medical aids and bills all medical aids directly; no medical aid scheme is excluded.
What are the fees?
